  Question:  How do I schedule a Consultation?

 

Answer:

Our main office schedules appointments for all of our clinic sites. Consultations with our physicians regarding your concerns may be arranged at any of our offices. We accept most insurance carriers, however, we always recommend that you verify your coverage. Please have your medical insurance information available when you call.

 

Question:

What can I expect on my first visit?

 

Answer:

Plan to spend at least one to one-half hours for the entire appointment. You will be completing paperwork and meeting with Patient Services and Nursing prior to seeing your physician. We ask that you bring all of your current medications, medical records, and current insurance card with you. Arriving promptly will facilitate your clinic visit.

 

Question:

Medication Refills

 

Answer:

Please call your pharmacy at least 3 days in advance of needing your medications refilled. The pharmacy will contact our office for the necessary information for your refill.

 

Question:

What Insurance Plans do you accept?

 

Answer:

We accept most Insurance Plans. When making your appointment, please check with the receptionist to confirm that we are providers for your insurance. If a referral is needed, please contact your primary physician and/ or insurance carrier to authorize the visit and to be sure you have coverage. With your authorization, we will submit all claims to your insurance on your behalf. You will receive a statement after your insurance has paid, indicating any balance you may owe. Some insurance policies restrict the use of hospitals and physicians. If you are scheduled for any procedures or tests, please notify your insurance company for prior authorization of coverage. We will ask to see your insurance cards at all visits.

 

Question:

Will I be given a bill on the day of my visit?

 

Answer:

Most Insurance plans have a co-pay and you are responsible for that. All accounts are due and payable within 30 days from the date of billing. If you are not insured, you must speak to the Business Office before your appointment. If, under special circumstances, you are unable to pay your bill, please contact our Business office at (763) 236-9198 to discuss payment arrangements. Co-payments must be paid at the time of service.
